How do I get a copy of a birth certificate?
If you were born in the Town of Millis or if you parents lived in Millis at the time of your birth, you may obtain a copy of your birth certificate from the Town Clerk’s office.  The fee is $10 per certified copy.  If you call ahead, the certificate will be ready when you come to pick it up.  If you request a copy of the certificate by mail, you must include a check made payable to the Town of Millis.

Also, please include a self addressed envelope and document will be mailed to you.

How do I get a copy of a death certificate?
Certified death certificates can be obtained for $10.00 per copy.  The Town Clerk’s office retains death certificates for persons who were residents of Millis at the time of death or for non-residents whose death occurred in Millis.

How do I get a copy of my marriage certificate?
Certified marriage certificates can be obtained for $10.00 per copy.  If you filed your Marriage Intentions in Millis, your certificate would be here.  However, if you were married in Millis but filed your Intentions in another town, you must obtain your certificate from the town where you originally filed, or from the Massachusetts Registry of Vital Records in Dorchester.
